The 2010 requests for applications are CLOSED. Mailing has been finalized

and the field is full. Please try again for next year after April 17, 2010.

At that time send in a Self Addressed Stamped Envelope (business size). To

make it fair, all requests for applications will be mailed out exactly at

the same time sometime in February 2011 (maybe earlier or later since a

mailing date has not been determined yet). We accept up to the first 1500

correctly filled out applications that are returned. Some send in prepaid

self-addressed USPS overnight mail to receive their application and return
